Reimbursement Request
Galatians 6:10 "So then, while we have opportunity, let us do good to all people, and especially to those who are of the household of the faith.
Philippians 2:3-4 "Do nothing from selfishness or empty conceit, but with humility of mind regard one another as more important
than yourselves; 4 do not merely look out for your own personal interests, but also for the interests of others.
This form is to be used when a member of Southside Tabernacle Baptist Church requests reimbursement of funds from Southside Tabernacle Baptist Church for purchases which the SSTBC member made for the church, acts of benevolence on behalf of Southside Tabernacle Baptist Church involving personal funds, or for the purchase of meals during times of counseling or evangelistic encounters. A copy/copies of all receipts related to the reimbursement request must be submitted with this reimbursement request form. All other instances of SSTBC ministry related purchases should be listed and requested via the Ministry, Event and Activity Request form at the time of the planning.
